Home Nursing in Riyadh 2026: Legal Routes & Costs

If you are searching for home nursing in Riyadh, you are probably choosing between two completely different legal routes — and most families do not realise.

If you are searching for home nursing in Riyadh, you are probably choosing between two completely different legal routes — and most families do not realise they are different until something goes wrong.

One route is a licensed home healthcare provider, regulated by the Ministry of Health. The other is a domestic caregiver recruited through Musaned under the Ministry of Human Resources and Social Development. They involve different permissions, different paperwork, and critically, different limits on what the worker is legally allowed to do.

This guide explains which route fits your situation, how to verify either one, and the mistakes that expose Riyadh families to fines or unsafe care.

The two routes, side by side

Route 1: MOH-licensed home healthcare

This is clinical care delivered at home by qualified nurses or therapists working for a facility licensed by the Ministry of Health. Think wound dressing, injections, catheter and feeding-tube management, post-surgical recovery, ventilator support, or physiotherapy.

Staff are registered health practitioners. The provider — not you — is the employer, and the provider carries the clinical liability. Visits are usually scheduled by the hour or by the visit rather than live-in.

You can confirm a provider's licence status directly with the Ministry of Health at moh.gov.sa, and check that individual practitioners hold a valid registration with the Saudi Commission for Health Specialties.

Route 2: A domestic caregiver via Musaned

This is non-clinical personal care: help with bathing, dressing, mobility, meals, medication reminders, companionship and household support. The worker lives in your home on a domestic worker visa, and you are the employer.

Recruitment runs through Musaned, the official platform of the Ministry of Human Resources and Social Development. If you are new to the platform, our explainer on what Musaned is and how it works covers the basics, and our full guide to hiring an elderly caregiver in Saudi Arabia walks through the contract and approval steps.

The line you must not cross

This is the single most important point in this guide.

A domestic caregiver on a domestic worker visa is not a licensed health practitioner. Asking them to perform clinical tasks — injections, IV lines, wound care, changing catheters, operating medical equipment — puts your relative at genuine risk and places the task outside what that worker is permitted and insured to do.

Families often slide across this line gradually. A caregiver hired for bathing and meals is asked to "just do the insulin" because the nurse visit is expensive. That is the moment the arrangement stops being compliant.

If your relative's needs include anything clinical, you need Route 1 for those tasks — even if a domestic caregiver handles everything else.

Many Riyadh families use both

The most common sensible setup for a dependent elderly parent is a live-in domestic caregiver for daily personal care and companionship, plus scheduled visits from an MOH-licensed provider for the medical component.

This tends to cost less than round-the-clock clinical nursing while keeping the clinical work in licensed hands.

What it costs

Licensed home nursing in Riyadh is generally priced per visit or per hour, and rates vary considerably depending on the clinical complexity involved, the hours required, and whether you need overnight or weekend cover. Ask any provider for a written schedule of charges before committing.

A domestic caregiver recruited through Musaned works differently: there are one-off recruitment and visa costs, then an ongoing monthly salary. Recruitment costs depend heavily on the worker's nationality and the agency involved — our breakdown of Musaned domestic worker fees in Saudi Arabia sets out the components, and typical monthly salary ranges by nationality are covered separately.

Salaries are agreed in the Musaned contract, and some sending countries set minimums for their nationals. Official fees are reviewed periodically, so always confirm current figures on musaned.com.sa rather than relying on a figure quoted to you verbally by an agency.

How to verify before you commit

For a home nursing provider

  • Ask for the facility's Ministry of Health licence number and verify it independently.
  • Confirm the specific nurse assigned to you is registered with the Saudi Commission for Health Specialties.
  • Get the scope of care in writing — which tasks are covered, which are not.
  • Ask who to contact out of hours, and what happens if your assigned nurse is unavailable.

For a domestic caregiver

  • Use only a recruitment office licensed by the Ministry of Human Resources and Social Development, and check it is active on Musaned.
  • Issue the contract through Musaned — never on a side agreement.
  • Ask specifically about elderly-care experience and training, not just general housekeeping.
  • Pay salary electronically through Musaned's approved wage-transfer channels. Since 1 January 2026 this is mandatory for all domestic-worker employers.

You can browse licensed recruitment offices across Saudi Arabia and the wider GCC in our verified agency directory. If you are hiring domestic help in the capital more broadly, our guide to hiring a maid in Riyadh legally covers the same approval chain.

Red flags

Walk away if a provider or office cannot produce a licence number, pressures you to pay cash with no receipt, offers a "nurse" who cannot show an SCFHS registration or who is on a housemaid profession, or proposes to transfer someone already working for another employer without a documented transfer. (Musaned does list "home nurse" and "physical therapy specialist" professions, but a worker on those visas still needs SCFHS classification and registration before doing clinical work. A housemaid or caregiver visa never covers it.)

That last one matters more than people expect. An improperly transferred worker can leave you exposed to a dispute you did not create.

Frequently asked questions

Can a housemaid legally care for my elderly parent in Riyadh?

Yes, for non-clinical personal care — bathing, dressing, mobility, meals, medication reminders and companionship. She cannot perform medical procedures. If your relative needs clinical care, that must come from a Ministry of Health–licensed provider.

Is home nursing cheaper than hiring a live-in caregiver?

It depends entirely on how many hours you need. Occasional licensed nursing visits are typically cheaper overall than employing a full-time live-in worker, but continuous clinical nursing is usually considerably more expensive than a domestic caregiver's monthly salary. Compare total monthly outlay, not headline rates.

Do I need Musaned if I go through a home nursing company?

No. Musaned applies when you are the employer of a domestic worker. With a licensed home healthcare provider, the company employs the nurse, so you are buying a service rather than sponsoring a visa.

Can I upgrade my current housemaid into a caregiver role?

You can broaden her duties into elderly personal care, and training genuinely improves the quality of care. Our guide on moving from housemaid to elderly caregiver in Saudi Arabia covers the skills and free course options. This still does not permit clinical tasks.

What if my relative's needs change and become medical?

Add a licensed provider for the clinical component rather than asking your domestic caregiver to absorb it. Keeping the two routes separate is what protects both your relative and your legal position.

Getting it right

Most Riyadh families searching for home nursing actually need a combination: licensed clinical visits for the medical work, and a well-chosen domestic caregiver for everything else. Verify both through the official channels, keep the clinical line clear, and get every scope of care in writing.
